Covid-19 Health Declaration

  • For the protection of all staff, contractors and visitors who attend work in the office and to prevent the spread of the virus, we are asking everyone to complete the following health declaration prior to attending site.

    All employees, and visitors must complete the Declaration each day before they come to the workplace, to confirm that they have not knowingly been in contact with an infected person, have been maintaining social distancing and do not have symptoms themselves.

    Please submit the form either on the morning of your attendance or on the previous evening.

    If you cannot answer positively any of the questions below, Do Not attend work and contact your manager as soon as possible.
